Leading Awareness To action Through Implementation of Cardiometabolic Effort (LATTICE™) Consortium is a first-of-its-kind community dedicated to addressing cardiometabolic patient needs through implementation efforts. By sharing collectively-learned tools and methodologies, we can improve the effectiveness of cardiometabolic patient care at scale.
A network of independent experts with a shared goal to address cardiometabolic patient care through evidence-based tools and methodologies, the consortium is led by its experts who work together to develop our programs and activities. The LATTICE™ consortium is structured so that there is no hierarchy—each expert’s opinion is valued and equally considered.
